The Health Insurance Portability and Accountability Act (HIPAA) includes a Privacy Rule that became effective on April 14, 2003. It requires that employers and health plans protect and secure patients' identifiable health information by controlling who has access to health information and what happens to it.
You will need to complete a HIPPA authorization form if you want to designate someone to have access to your health records.
